Extract (don't paraphrase): one-line decision, rejected alternatives, checkable claims, new vs already-exists, author's open/to-confirm markers, numbers/owners/deadlines. Keep author's open items and your concerns in separate sections; every concern points at a specific line; "likely" stays "likely". Identifiers stay verbatim; follow source section order. Inferred open items get labeled as yours. Tip only (no invented prompt). Not affiliated with xAI.
